Process includes:

• Chart Selection based on the predetermined parameters of the audit
• Audit focus is on completeness and accuracy of selected diagnosis and intervention codes and/or quality of clinical documentation
• Provision of meaningful and authentic documentation for all recommendations for improvement
• Review of all findings with client coding personnel and provision of a rationale for recommendations
• Report of findings and recommendations
